Abstract

Patient Safety is a global and were gathered to become the national issue, an important respondences of this study .The results of the study showed that there was no component of the quality of the health services , as a basic principle in patient care as well as a critical component of quality management significant relationship between age , sex , length of employment , in the educational background, knowledge level , colleagues attitudes with the Patient Safety incident reports, but the attitude of the leader (head nurse) was associated with the reporting of Patiens Safety Incidents with p value = 0,011 . The study then recommended that the hospital management should always inform Patient Safety programs through providing some trainings about the importance of incident reporting , workflow , and reporting methods hospital.The attenttion of the public on patient safety is driven by the high number of incidence on Adverse Event ( AE ) in the hospital. The incidence reports is the beginning of the learning process that can lead to decreasing level and prevention on reoccuring the incidence . The problem is that the AE cases are rare to be reported and if it is reported then only on serious/severe cases. The cases of mild injury or Near Miss (NM) is almost never be reported . This study was aimed to explore the causal of the low reporting on incident reports at Panti Rapih hospital .Using a cross - sectional approachment and accidental sampling tecnique, 68 nurses to the analysis and implementation of all nurses and the head nurse should be given refreshing / training on leadership , particularly in relation to how to build a culture of safety for every department.
