Abstract
Application of Cold Compress Therapy to Reduce Acute Pain in Patients with Closed Fractures in the Emergency Department of Tzu Chi Hospital Jakarta Background: Acute pain is one of the major problems experienced by patients with closed fractures during the initial phase of treatment in the Emergency Department (ED). Inadequately managed pain may interfere with patient comfort and mobility and may affect physiological responses. Pain management commonly relies on pharmacological therapy; however, non-pharmacological interventions can be used as supportive therapy. Cold compress therapy is a non-pharmacological intervention that may help reduce pain intensity through vasoconstriction, reduction of edema and inflammation, and slowing of nerve impulse transmission. Based on observations in the Emergency Department of Tzu Chi Hospital Jakarta, cold compress therapy had not been routinely implemented as part of nursing interventions for patients with closed fractures. Objective: This case study aimed to describe the application of cold compress therapy in reducing acute pain intensity in patients with closed fractures in the Emergency Department of Tzu Chi Hospital Jakarta. Methods: This study used a descriptive case study design with a nursing process approach. The subjects consisted of four patients with closed fractures who met the inclusion criteria and were selected using purposive sampling. Pain intensity was assessed using the Numeric Rating Scale (NRS) before and after the intervention. Cold compress therapy was administered using a coolpack for approximately 20 minutes to the injured area. The case study was conducted in the Emergency Department of Tzu Chi Hospital Jakarta from March 16 to March 28, 2026. Results: The assessment before the intervention showed that all patients experienced acute pain with an intensity ranging from 5 to 7, categorized as moderate to severe pain. After approximately 20 minutes of cold compress therapy using a coolpack, pain intensity decreased in all patients. The first patient’s pain score decreased from 6/10 to 3/10, the second patient from 7/10 to 3/10, the third patient from 5/10 to 2/10, and the fourth patient from 7/10 to 4/10. Overall, pain intensity decreased from a range of 5–7 to 2–4, with a reduction of 3–4 points in each patient. Patients also reported increased comfort, and no increase in swelling was observed following the intervention. Conclusion: The application of cold compress therapy using a coolpack for approximately 20 minutes demonstrated a reduction in pain intensity among all patients with closed fractures in this case study. Cold compress therapy can be used as a supportive non-pharmacological nursing intervention to help reduce acute pain and improve comfort in patients with closed fractures during the initial phase of treatment in the Emergency Department. However, the findings should be interpreted cautiously because of the limited number of subjects, short observation period, absence of a control group, and subjective nature of pain assessment.
